Mark Heffer
CO-FOUNDER & MANAGING PARTNER

Mark Heffer is the Co-Founder and Managing Partner of AxisMD Advisory, where he partners with healthcare organizations, providers, and investors to improve operational performance, accelerate growth, and build scalable care delivery models.
With executive leadership experience spanning physician practices, anesthesia, cardiology, allergy, dental, and behavioral health, Mark has built a reputation for translating complex operational challenges into practical, measurable strategies. His expertise includes practice operations, revenue cycle management, payer contracting, mergers and acquisitions, provider compensation, strategic growth, and healthcare market expansion.
Throughout his career, Mark has served in senior executive roles, including President, Chief Operating Officer, Chief Executive Officer, Vice President of Operations, and Market President. He has led organizations ranging from independent physician groups to private equity-backed healthcare platforms, overseeing multi-site operations, provider networks, clinical service lines, and large-scale growth initiatives.
At AxisMD, Mark focuses on helping healthcare organizations strengthen operational infrastructure, improve financial performance, expand market access, and develop sustainable growth strategies. He is particularly passionate about supporting physician-led organizations as they navigate an increasingly complex healthcare environment.
Mark believes that successful healthcare organizations are built by aligning providers, operations, finance, and patient experience around a shared vision. His leadership philosophy centers on transparency, accountability, and creating systems that allow clinicians to focus on what matters most: delivering exceptional patient care.
Based in Dallas, Texas, Mark enjoys spending time with his wife and two daughters, traveling, and remaining actively involved in his church and local community.
Sara Sommers
CO-FOUNDER & MANAGING PARTNER

Sara Sommers is the Co-Founder and Managing Partner of AxisMD Advisory, where she works with independent physician practice owners, health systems, and investors to strengthen operational performance, evaluate partnership opportunities, and protect physician interests in complex transactions.
With executive leadership experience spanning cardiovascular, pediatric, neonatal, renal, and multi-specialty physician enterprises, Sara has built a reputation for turning operational and financial complexity into decisions physicians can act on. Her expertise includes practice operations, physician compensation design, private equity and MSO partnership evaluation, mergers and acquisitions, revenue cycle performance, value-based care economics, service line development, and multi-site growth.
Most recently, Sara served as Regional President for South Texas at US Heart & Vascular, directing strategic operations and P&L management for a multi-site cardiovascular service line generating more than $150 million in annual revenue across 50 physicians and over 300 employees. She expanded margin 50% year over year and grew ancillary revenue 500% by building out diagnostic imaging, ambulatory surgery, and vascular access services. She opened two cardiac ambulatory surgery centers with a third in progress, added more than ten PET/CT units across the market, and rebuilt revenue cycle performance at multiple practices — reducing days in A/R from 45 to 32 and lifting net-to-expected revenue from 84% to 92% in six months.
Earlier, as Senior Director of Value-Based Care Market Operations at US Renal Care, Sara led the P&L for Texas value-based joint venture partnerships and met every quality and risk-adjustment goal of the CMS program in her first year. At Mednax/Pediatrix she was the market executive over ten physician practices spanning 30+ ambulatory locations, ten hospital-based services, and more than 100 practitioners across maternal-fetal medicine, neonatology, pediatric surgery, OB/GYN, and pediatric cardiology. At Texas Children's Hospital she managed budgets for neonatology and renal services covering 150+ clinicians, 600+ FTEs, 187 NICU beds, and more than $500 million in revenue.
Across those roles, Sara has led more than 25 transactions ranging from $250,000 to $40 million, from initial diligence through post-close integration — routinely bringing acquired practices to cultural alignment, operational readiness, and financial performance within the first 90 days. Her record is built on growth that holds: new service lines taken from financial model through staffing, credentialing, and payer enrollment; de novo sites opened and integrated; and provider productivity raised 10% in RVUs per physician. She does it by partnering closely with physicians rather than working around them — treating clinical leadership as a co-author of the operating plan rather than an audience for it.
At AxisMD, Sara focuses on helping physician owners understand the economics of their own practice before someone else does. She pays particular attention to the two places where a deal most often turns against the physician: how compensation is structured after close, and what management fees are actually buried in the MSO agreement.
Sara believes physicians deserve advisors who sit on their side of the table without exception. Her approach centers on transparency, plain-language analysis, and giving practice owners the same caliber of financial and operational insight that platforms and investors bring to the table.
Based in Houston, Texas, Sara is a competitive tennis player and served on the board of the Houston Tennis Association from 2022 to 2025. She mentors healthcare MBA students at Baylor University and spends her time outside of work with her husband and their children.
